How to request an appointment
Doctors’ Appointments
The quickest and easiest way to request a doctor’s appointment is through our medical request form on our website or through NHS APP and website. Our forms are open from 7am to 3pm Monday to Friday excluding bank holidays.

If you are unable to use our online form, you can call the surgery and our reception team will complete the form for you. We treat all requests in the same way, no matter how you contact us. Your care and safety are our first priority. A doctor will carefully review all requests and we will get back to you the same day with a response.
The form is sometimes switched off for safety reasons. In this situation and when we are closed please call NHS111 or go to 111.nhs.uk.
Nurse Appointments
Please call reception on 01483 400200 to request a nurse appointment.
Types of appointment
We offer the following types of appointments:
Urgent appointments
Please tell us if you feel that you need an appointment urgently. We'll review your request carefully. Where appropriate, we'll offer you an appointment on the same working day, or the next day. If we think you need immediate help, we may tell you to go to a hospital.
When the surgery is closed, please call 111 or go to 111.nhs.uk.
Routine appointments
If you don't need immediate help, please let us know when you request an appointment. This may be, for example, because:
- you're due to have a health check-up or screening
- you need a vaccination
- your ailment is minor, or you've had it for a long time already
Where appropriate, we'll arrange an appointment for you in the next two weeks. If you've asked to see a specific person, there may be a longer wait where they are fully booked or away.
Evening and weekend appointments
We provide evening and weekend appointments outside of our normal opening hours. You can request these by contacting the surgery or by calling 111.
Home visits
If you cannot leave your home, please let us know when you request an appointment.
Help during your appointment
When you request an appointment, please tell us if you need specific help, for example:
- if you cannot come to the surgery
- if you need an interpreter
- or if you have any other access or communication needs
Cancel an appointment
If you cannot come to your appointment, please cancel it as soon as possible so that we can help another patient. You can cancel an appointment:
- by following the link in the appointment confirmation
- through the NHS app or website
- or by phoning the surgery and using the automated check and cancel system or speaking to a reception team member
- or by using our online form
Urgent advice: Call 111 if you need urgent advice
NHS 111 can help if you have an urgent medical problem and you’re not sure what to do. To get help:
- go to 111.nhs.uk (for people aged 5 and over only)
- call 111
Available 24 hours a day, 7 days a week.
Immediate action required: Call 999 or go to A&E in an emergency
Medical emergencies can include:
- loss of consciousness
- an acute confused state
- fits that are not stopping
- chest pain
- breathing difficulties
- severe bleeding that cannot be stopped
- severe allergic reactions
- severe burns or scalds
