Long Term Condition Reviews

We have implemented a new process to ensure we review annual long term conditions in a more efficient manner to reduce the number of annual appointments patients with long term conditions require across the year. As of November 2024, we will be recalling patients by their birth month to book an appointment with our Health Care Assistant or Practice Nurse. If your GP feels you need a follow up after this appointment you will be contacted by the practice.

We appreciate your valuable time and we wanted to simplify the process for you to avoid you coming into the practice multiple times a year to attend separate long term conditions reviews.
